• Captain's meeting will be held on Thursday, June 28, 2012 at 7:00 pm for all pre-registered teams
• TEAM FEE:  $1,300
  $500 non-refundable deposit due with registration; balance of $800 due no later than Tuesday, July 24, 2012  NO EXCEPTIONS
• Individual Fee:  $150
  Total amount due with registration
• Make checks payable to SUBURBAN HOCKEY.  A $40 fee will be assessed on all NSF checks
• Referees and scorekeepers included in league fees
• Registered teams supply their own jerseys.   Individual draft teams will be provided jerseys
• All players must be registered with USA Hockey.  Players who were not registered for the fall/winter or spring season MUST register themselves online at www.usahockeyregistration.com using a credit card.  A registration confirmation is required for all players as proof of their USA Hockey insurance, except for those who played at the Onyx during the fall/winter or spring season
• All participants are required to sign a release of liability waiver
• Suburban Sports Group reserves the right to move teams to the proper level if their caliber of play is inappropriate to the standard of the league entered
